    Most supplement companies exist to sell products. Dr. Sanjeev Goel, M.D., Canada’s Happiness and Longevity doctor, founded Peak Human because he couldn’t find products he trusted enough to take himself.

    That distinction matters when evaluating NMN supplements, a growing category within the longevity space and also one of the most misunderstood. Dr. Goel, a family physician with more than 20 years of clinical experience and a specialty in longevity medicine, takes NMN as part of a multi-compound daily stack. He is also quick to explain exactly what the research does and does not show.

    «I won’t promote things without proof or the possibility of being true,» Dr. Goel has said of his approach to supplementation. That standard, applied, according to Dr. Goel, across approximately 250 patients and his own daily protocol, is the lens through which he evaluates every compound that enters his formulations.

    What is an NMN supplement and the NAD+ longevity connection?

    NMN stands for nicotinamide mononucleotide, a naturally occurring molecule that serves as a direct precursor to NAD+ (nicotinamide adenine dinucleotide). NAD+ is a coenzyme found in every living cell. It powers the mitochondria that generate cellular energy, facilitates DNA repair, and activates sirtuins, a family of proteins often described as longevity genes.

    The problem is that NAD+ levels can decline significantly with age. Research has documented that NAD+ concentrations in human tissue can fall by as much as 50% between young adulthood and midlife. That decline is now considered a hallmark of cellular aging, contributing to reduced energy production, impaired DNA repair, and increased cellular dysfunction.

    Supplementing with NMN is one proposed strategy for restoring NAD+ levels. Animal studies, particularly in mice, have suggested potential benefits: improved mitochondrial function, extended lifespan, enhanced muscle endurance, and better metabolic markers. Human clinical trials are emerging that show potential effects on NAD+ levels in blood, though evidence of downstream longevity benefits in people remains an active area of research.

    Dr. Goel’s position is characteristically honest: the mechanistic rationale is strong, the mouse data is compelling, and the human biomarker data is encouraging. What does not yet exist is a decades-long human trial proving that NMN supplementation extends healthy lifespan. He takes it as part of his longevity stack because the compound fits within his broader framework of evidence-informed, personally tested protocols.

    NMN vs NAD: Understanding the precursor pathways

    A common question among people investigating NAD+ supplementation is why not simply take NAD+ directly. The answer lies in biochemistry. Oral NAD+ does not survive digestion intact in meaningful quantities. The molecule is too large and chemically unstable to cross the intestinal wall and reach cells where it is needed.

    NMN and NR (nicotinamide riboside) are the two main precursors that the body converts into NAD+ intracellularly. Both have research support, but they operate through different pathways. NR enters cells through specific transporters and is converted to NMN before becoming NAD+. NMN, by contrast, is thought to have a dedicated transporter called Slc12a8 that allows it to enter cells more directly, potentially offering faster and more efficient conversion, though this mechanism is still being studied in human tissue.

    Dr. Goel chose NMN over NR for his Fasting Pro formulation, a 12-compound supplement he developed through personal experimentation and clinical practice. His rationale combines the transporter evidence with his own subjective response over time.     On dosing, Dr. Goel takes NMN in the morning, often on an empty stomach or early in his eating window, which begins around 10 or 11 a.m. as part of his time-restricted eating practice. While isolating the specific contribution of NMN within a multi-compound protocol is difficult, his overall aging speed reflects a protocol he has refined over years.

    Dr. Goel’s approach to mitochondrial support

    NMN does not act in isolation in Dr. Goel’s protocol. He approaches mitochondrial health as a system, selecting compounds with distinct and complementary mechanisms.

    CoQ10 (coenzyme Q10) supports the electron transport chain within the mitochondria, directly contributing to ATP production.     The stack is designed to work with, not instead of, lifestyle fundamentals. Zone 2 cardiovascular training, meaning sustained aerobic exercise at roughly 60 to 70% of maximum heart rate, is one factor that research suggests may support mitochondrial biogenesis. Dr. Goel plays soccer with 25-year-olds as a regular part of his weekly routine, and he credits Zone 2 cardio with maintaining the mitochondrial density that supplements can then support and extend.

    Fasting is associated with autophagy, a cellular recycling process. Dr. Goel says Fasting Pro is designed to complement fasting through ingredients intended to influence pathways linked to fasting, including AMPK. He notes that NMN users may report insomnia, mild anxiety, or no noticeable effects, which he attributes to product quality and individual biology. He recommends independently verified products, lower starting doses, and tracking both subjective effects and objective measures.

    The supplements Dr. Goel won’t take and why

    The credibility of Dr. Goel’s supplement recommendations rests partly on what he declines to endorse. Several popular longevity supplements that generate significant industry revenue do not make it into his protocol.

    Resveratrol illustrates the gap between promising laboratory and animal findings and proven benefits in humans: standard supplements may not reproduce the reported effects, and poor bioavailability remains a concern. Dr. Goel uses a particular resveratrol form in Fasting Pro but does not generally recommend typical retail products. He places many adaptogens in a similar category—plausible mechanisms but limited human evidence, potentially overstated claims, and possible interactions. His approach is not anti-supplement; it is evidence-focused.

He also compares a product’s dose with the doses studied in human research, since products may be underdosed relative to trials or exceed doses with established safety data.

    Funding and study design can reveal bias. Weak evidence often involves industry-funded, unreplicated trials, surrogate endpoints without clinical-outcome data, or testimonial-based marketing. Other red flags can include proprietary blends that hide doses, multi-level marketing incentives, and «miracle» claims that overstate what any single compound can achieve.

    Dr. Goel recommends adding supplements one at a time, waiting four to eight weeks between changes, and tracking both subjective effects and objective measures such as bloodwork. He advises stopping products that provide no measurable benefit and treating supplements as additions to core lifestyle habits. He says biological-age testing places his biological age below his chronological age, which he attributes mainly to long-term practices such as time-restricted eating, Zone 2 cardio, alcohol avoidance, sleep consistency, hormone management, and mindfulness—not supplements such as NMN.
